Rare Case of Meningoencephalitis Due to Aspergillus and Acanthamoeba Coinfection

چکیده
ABSTR ACT: Aspergillus and Acanthamoeba cause central nervous system (CNS) infection mostly in immunocompromised hosts, disseminating via the hematogenous route from a primary focus in the respiratory tract. We are presenting a case of an immunocompetent patient diagnosed with Aspergillus and Acanthamoeba coinfection of the CNS, a rare finding. A positive cerebrospinal fluid (CSF) galactomannan antigen test confirmed the presence of Aspergillus, and direct visualization in CSF sample and culture confirmed the presence of Acanthamoeba.

Unexpected postmortem diagnosis of acanthamoeba meningoencephalitis in an immunocompetent child.
Meningoencephalitis caused by Acanthamoeba spp. is a rare opportunistic infection, difficult to diagnose and treat, which causes death in almost all cases. Here, the authors report a 5-year-old Iranian immunocompetent girl who died of fulminant acanthamoeba meningoencephalitis. To the authors' knowledge, this is the first case of acanthamoeba meningoencephalitis in Iran.

We describe a 66-year-old woman with therapy-refractory cryoglobulinemia treated with rituximab, plasmapheresis, and steroids; a case of fatal meningoencephalitis caused by Acanthamoeba spp. then developed. Such infections are rare and show an unusually rapid course (possibly related to rituximab).

Introduction Definition Free-living amebae of the genera Naegleria, Acanthamoeba, and Balamuthia cause fatal diseases of the central nervous system (CNS) of humans.1-33 Naegleria fowleri causes an acute and fulminant primary amebic meningoencephalitis (PAM) in children and young adults with a history of exposure to fresh water leading to death within 5 to 10 days after the onset of symptoms.4-6...
متن کاملRetrospective identification of Acanthamoeba culbertsoni in a case of amoebic meningoencephalitis.
Acanthamoeba culbertsoni was identified retrospectively in a case of amoebic meningoencephalitis, previously reported by Jager and Stamm (Lancet, 2, 1343, 1972). This is the second report of this species causing secondary infection in man. Positive results were obtained only with anti-A. culbertsoni sera when the brain sections were stained by the indirect immunofluorescence antibody test with ...

The incidence of protozoal and helminthic infestations of the central nervous system (CNS) is less than 1%, but these infestations tend to follow a fatal course. They are more common among children, the elderly and immunocompromised individuals. CNS infections due to Entamoeba histolytica have been known for a long time. In recent years, especially in developing countries, there has been an inc...
